For a student to attend a District 129 school, the student and parent/guardian must be a resident of the District. Enrollment takes place at the school where the student will attend which is determined through boundaries determined by the District 129 Board of Education.

For registration purposes, all students must be accompanied by a parent or guardian at the time of registration and provide proof of residency in District 129. All students must have an official birth certificate and show proof of physical examinations and immunizations as required by Illinois State Law.

State guidelines require that children must be five years of age on or before September 1 in order to be eligible to attend kindergarten.

Immunizations required by the State of Illinois are as follows:

Kindergarten: Physical Examination required. Diphtheria, Tetanus, Pertussis Series (3 infant injections and one booster after age 4 years);  Oral Polio immunization series (2 infant oral plus one booster after 4 years); MMR (Measles/Mumps/Rubella): one immunization given at or after 12 months of age; a second measles immunization required that was given at least one month after initial injection (recommended that entire MMR be given as second injection).  A varicella vaccine or MD documentation of history of disease.

Fifth Grade Physical Examination required.

Fifth Grade Hepatitis B vaccine (series of 3 immunizations)

Ninth Grade Physical Examination required. Diphtheria/Tetanus booster (required at 10 year interval after last injection).

Dental examinations are required in kindergarten, second, and sixth grade.

All students must comply with the state law regarding physical examination and immunizations by the first day of school each year or they will be excluded from school until in compliance. Please note that this is a change from the policy of previous years.

Students new to District 129 will be given 30 days from the date of registration to obtain previous school records and/or show evidence of a physical examination completed within the last calendar year as well as up-to-date immunizations.

Health forms are available at all schools or at the District 129 Administration Office, 80 S, River Street, Aurora, Illinois 60506
