PLEASE READ THE FOLLOWING INSTRUCTIONS CAREFULLY
There are several required new employee forms that we will ask you to complete in advance of your scheduled fingerprint appointment here at the Administration Office. We ask you to do this ahead of time in the interest of saving time at the meeting. Most of these forms can be completed "on-screen" and then printed out. You will be given brief instructions for each of the forms. If you have any questions as you are completing forms, please contact us simply by calling; 630-301-5030.
*Please note: Substitute applicants must complete fingerprinting and background checks through the Kane County ROE located in Geneva. You may visit http://www.kaneroe.org/fingerprinting/ to make an appointment at your earliest convenience.

Federal W-4 Form & Instructions

State of Illinois Withholding Form (W-4)
The Teacher Retirement System is your mandatory retirement plan. The school board contributes your portion towards your retirement. Instructions to complete this form are included with the form.
TRS Enrollment Form
This is a federal form. You only need to complete & sign Section 1 on page 7. You will be asked at the meeting to provide your driver's license and social security card to prove eligibility for employment in The United States.
Employment Eligibility Form (I-9)
You will be required to turn in a physical form. Although it isn't necessary to have this completed at your fingerprint appointment, having this form will give you an opportunity to make an appointment now if you choose to do so.
Employee's Health Examination Record
If you have any prior teaching experience, please complete this form. Complete the top half only, and turn it in with your other paperwork. District 129 will forward it on to your prior school/s for verification of employment.

If you work for an employer who does not withhold Social Security taxes from your salary, such as a government agency like a school district or an employer in another country, the pension you get based on that work may reduce your Social Security benefits. The Windfall Elimination Provision affects how the amount of your retirement or disability benefit is calculated if you receive a pension from work where Social Security taxes were not taken out of your pay. A modified formula is used to calculate your benefit amount, resulting in a lower Social Security benefit than you otherwise would receive.
