Certified Staff Pre-Hire Documentation

PLEASE READ THE FOLLOWING INSTRUCTIONS CAREFULLY

There are several required new employee forms that we will ask you to complete in advance of your scheduled fingerprint appointment here at the Administration Office. We ask you to do this ahead of time in the interest of saving time at the meeting. Most of these forms can be completed "on-screen" and then printed out. You will be given brief instructions for each of the forms. If you have any questions as you are completing forms, please contact us by calling; 630-301-5030.

Please complete, print, and bring with you the following forms:

Federal W-4 Form & Instructions

State W-4 Form & Instructions

Employment Eligibility Form (I-9)
Instructions
(You only need to complete & sign Section 1. You will be asked at the meeting to provide your driver's license and social security card to prove eligibility for employment in the United States)

Employee's Health Examination Record
(You will be required to turn in a physical form. Although it isn't necessary to have this completed at your fingerprint appointment, having this form will give you an opportunity to make an appointment now if you choose to do so)

Mandated Reporter Training
(Please print out the certificate of completion and bring it with you along with the other forms)

CERTIFIED POSITIONS NOT COVERED BY SOCIAL SECURITY

If you work for an employer who does not withhold Social Security taxes from your salary, such as a government agency like a school district or an employer in another country, the pension you get based on that work may reduce your Social Security benefits. The Windfall Elimination Provision affects how the amount of your retirement or disability benefit is calculated if you receive a pension from work where Social Security taxes were not taken out of your pay. A modified formula is used to calculate your benefit amount, resulting in a lower Social Security benefit than you otherwise would receive. 5 STEPS TO BECOMING A SD129 SUBSTITUTE

(For roles such as teacher, paraprofessional, office professional, and health assistant)

CERTIFIED POSITIONS NOT COVERED BY SOCIAL SECURITY

STEP 1 Begin by obtaining the appropriate licensure, if needed. Did you know there is a SHORT-TERM Substitute License also? Here is more information *No license is required* for non-certified substitutes such as office professionals and health assistants. STEP 2 Next, apply for one of the SD129 substitute roles online.	Substitute Teacher Application Substitute Paraprofessional Application Substitute Office Professional or Health Assistant Application
STEP 3 Then, schedule and complete the fingerprinting process with the Kane County Regional Office of Education.	Schedule here
STEP 4 Once complete, communicate the date of your fingerprinting session and share your desire to become a substitute with Human Resources.	Contact Information: subs@sd129.org 630-301-5025
