Health Services
Physical examination and immunizations are required for school entrance in specific grades according to Illinois Code. All documentation should be submitted to your school nurse as soon as possible.
PER DISTRICT 129 POLICY 7.1, THE DISTRICT EXCLUDES STUDENTS FOR HEALTH EXAMINATIONS AND IMMUNIZATION REQUIREMENTS on THE OCTOBER 15 EXCLUSION DATE OF THE CURRENT SCHOOL YEAR.
Other Serious Medical Conditions
Contact your school nurse immediately if you have questions or if your child has a medical condition that will impact their school day. Additional information may be needed to ensure your child’s comfort, safety and well-being at school.

Please see medication forms for details regarding District 129’s Medication Policy. Medication Authorization forms must be renewed every school year.

2026-2027 Student Health Requirements
Physical and immunization requirements need to be submitted on or before October 15, 2026 to alleviate the risk of school exclusion.
