Self-Carry Medication Policy and Procedures

Only Authorized for Albuterol, Epinephrine, Insulin & Glucagon

 

Dear Parent/Guardian,

We provide the following information regarding student self-administration of medication at school. Students who are permitted to self-administer medication must meet the following requirements:

 

  1. The student’s Parent/Guardian must notify the school nurse or school health assistant that the child will self-carry medication.
  2. Prior to self-carrying any approved medication at school, the parent/guardian must complete and submit the required Medication Authorization Form for West Aurora School District. If the prescription changes, a new, revised Medication Authorization Form must be submitted. The Medication Authorization Form is only effective for the current school year and will need to be renewed each school year. The only medication which does not require a physician signature is Albuterol (see Self-Administration of Asthma Medication Authorization Form).
  3. The medication must have the prescription label adhered to it indicating the child’s name, the name of the medication, the dosage, the administration instructions and the name of the physician who prescribed the medication.
  4. The student is permitted to carry their medication while in school or at a school-sponsored activity when necessary.
  5. It is recommended and best practice for the student to have an extra supply of their medication in the school’s health office for use in the event the student forgets to bring their medication to school on a particular day.
  6. Parent signature on the Medication Authorization paperwork indicates that your student will never share medication with another person. If a student is found abusing their medication or using it improperly, the student’s parent/guardian will be notified and the student may lose the ability to self-administer medication.
  7. If a student self-administers epinephrine or glucagon, the health office must be notified and 911 called to monitor the student’s condition. If a student self-administers albuterol or insulin and does not experience marked improvement, they must notify the school health office immediately.
